Summary:
Dr. Lee explained that osteotomy is a highly effective technique in rhinoplasty, particularly for narrowing a wide nose or correcting a deviated nasal bridge. While lateral osteotomy is commonly performed, it has several limitations, prompting exploration of alternative methods.

Traditional Transcutaneous (Through-the-Skin) Osteotomy:
- May leave visible scars
- Limited mobility for repositioning the nasal bone

Intranasal Osteotomy:
- Risk of mucosal damage
- Difficult to maneuver in the thick nasal root region
To overcome these challenges, Dr. Lee utilized a 3 mm straight osteotome to perform a new intranasal lateral osteotomy. Beginning from the outside and transitioning the osteotomy line upward and inward—akin to a “leap”—this approach helps avoid mucosal injury and provides greater precision in directing the osteotomy. Dr. Lee reported successful outcomes using this modified technique.
